Travel Question before Immigrating to USA

Timeline

Quick background: My wife a Pakistani citizen and I (US Citizen) are planning on going to Malaysia to celebrate her birthday before returning to USA for the first time for her (IR-1 visa). Her total process for visa approval took 2.5+ years, and she was given a 221g and ds-5535 to fill out after her interview at the ISL Embassy. My question is would us going to Malaysia together before coming to USA for her so she get can processed and officially become a LPR a bad idea? Would CBP give her extra scrutiny and/or can they even turn her away even with an IR-1 approved visa because she traveled before immigrating to USA and after DS-5535?

They would not turn her away because she stopped over in Malaysia

 

Denial of entry to IR-1s is rare. I am not  seeing any reason in what you have described for entry to be denied.
