I like Etihad for this because of CBP pre-clearance in Abu Dhabi. You will get processed by CBP in Abu Dhabi, and if there is any confusion about your extension letter, Etihad in Pakistan might have experience getting a hold of CBP in Abu Dhabi to answer questions.

Have a safe flight! I transited through Turkey about a month ago. Used GC + extension letter on Turkish Airlines. No issues whatsoever. However, I understand the risk of being denied was there for sure. Though I travelled within Europe and and I have a pretty strong passport to my advantage.

I do not think it should be. 
 

LPRs cannot be denied entry to the U.S. when arriving at a port on U.S. soil. Worst case, lack of an original extension letter will result in an I-193 fee.

 

Now given your port of entry will be Abu Dhabi, if you lose your extension letter, you might be cursing my advice to use that route. But then again, it is on you, not me, for losing such a valuable document.

 

ESTA, pre-clearance, API, no fly lists, etc are all designed to keep undesirable humans (whether they are U.S. citizens, American Samoans, LPRs, and other non citizens) out of the U.S.  Once on U.S. soil, Due Process gets in way of keeping undesirables out.

Because Abu Dhabi is a U.S. port of entry not inside U.S. territory. Therefore if you lose your documents, CBP can legally deny you entry.

There was no issue at POE - Boston Logan.  Just a 1+ hour long line when several planes landed around the same time.

None at all other than the long line.  They didn't even ask for it - I assume they pulled up her info on their computer.
