mismatch in name on birth certificate for I485

so my birth certificate is issued in the local language which is malayalam and it has a automatic english translation. My english translation is wrong and does not match my name on passport. 

 

i have my school certificates and older passport that matches my current passport. is it recommended that i use other names used option in the i485 application or should I provide personal affidavites?

 

Please let me know my options
