Visa for medical observerships

I am medical graduate from Pakistan, doing USMLE. I have to apply for visit visa to do my observerships next year in June. But the problem is my sister already resides there(citizen),an immigrant from Pakistan since 2015 and my mother is a green card holder. I have to apply for my visa now in order to get it before July 2024, right now I don't have my observership letters but Ill receive it soon. So should I wait for the letters or apply for visa as tourist visiting my sister. My another sister who is married here in Pakistan has her immigration in process through my sister who resides there. Same goes for my brother who lives here unmarried. So, Im worried if it will affect my visit visa process. I would like to know what are the keys I should look for while applying for my visit visa to get it accepted as soon as possible without much hassle.
