Update: CEAC says case approved today. Any idea what that means.

The consular officer has approved your application subject to final processing by the consular section that is necessary to issue your visa. When that processing is complete, the status will change to "Issued". However, a final screening step must still be completed prior to issuance that could lead a consular officer to conclude that you are not eligible for a visa. If that occurs, the consular officer will notify you that your application has been denied and your status will change to "Refused".

what is that final screening?

Seems pretty self-explanatory.  What you are looking for is "Issued" which means the visa has been put in your passport.

Unfortunately, you still need to wait.

I don’t mind waiting it’s just that final statement where there is additional screening required that could lead an officer to change the status to refused. Any idea what that is? I know it’s self explanatory I’m just asking if anyone has experienced something similar.

The consulates (CEAC) started using the term "Refused" in place of "Administrative Processing" in some instances to sort of show there is more work to be done or they are requesting additional information either from you or another department that works with the IV unit in the consulate before the visa can be issued.  "Refused" does not mean "Denied" if that happens.
