Hello everyone..When we sent my wife's vaccination worksheet to be transferred to form I-693 (Thanks to the VJ members by the way, for suggesting Dr. Baker in Texas. it saved us hundreds of dollars in unneccesary vacinnations and an 80 dollar fee for the form itself- with him, it was just 10 bucks!) but he sent us the I-693 supplement and an additional sealed envelope for immigration. I was under the impression we would only need the I-693. should we send the envelope too? or hold onto it for the interview? thanks!

I beleive that USCIS want the sealed envelope. The other one is a copy for yourself.

Often the doctors will give you a sealed one to mail off and then just a regular copy for your records :) That is what happened with us.

Same here - one for us (unsealed), one for 'them' (sealed envelope) - it's nice that they do this. :thumbs:

I had the same, and I was told that there is the same form I-693 in the sealed envelope and that is what you send to USCIS.

5 Feb 2007 NOA1

1 May 2007 NOA2

12 July 2007 Interview in Vancouver, RFE

17 July 2007 Visa in hand

18 July 2007 Arrived in the US

07 Aug 07 Married

29 Aug 07 Applied for AOS

20 Oct 07 Biometrics scheduled
