DS-260 Completed but could not sign!!! Please HELP!!!

My wife and I just completed the DS-260, which kept on having us keep entering their alpha/numerical code to get to the signature page. Unfortunately, it never took us to that page but accepted it as submitted. I realized after it was too late that it wouldn't let us proceed was probably because we changed my wife's name on Passport and entered that with our Case # and maybe that was why we kept getting a new alpha/numerical code, but by then am thinking it just submitted it without getting to next page and no signature. What should we do now? Below  is the confirmation.

I'm wondering if this problem was because of a new Passport # or the name change is why it wouldn't send us to the signature page? After the 4th try it accepted it as submitted.

 

Does anyone know how this can be fixed?
