Hello, my husband lost his DS-3025, we are in the process of sending out his AOS and I noticed it is required he sends this form.

USCIS has received the sealed envelope that contained this form, but we just don't have a copy to send with the AOS, will he have to go back to a civil surgeon to get it filled out all over again?

Email the panel physician and ask for another copy.

Whether you submit a photocopy of it or not, you still may or may not have to redo your medical.

It's up to the IO.

We emailed her and she said there is no electronic copy of it, she said she would look to see if there is another physical copy of it. 
 

Can omitting this form result in denial of AOS

No.

You'd get asked by your IO to provide a new medical after interview or you'd receive an request to get it done again before interview scheduled. 

The DS3025 isn't even required to be submitted as they already have your visa packet with the original copy in it. 

It's a RECOMMENDATION to submit a duplicate copy only.

 

PS. remove the word "denial" from your vocabulary and relax.

I suggest lots of reading up on the process in the forums and to join your monthly filing thread so you can get a clear picture of how easy this is and how rare K1s are denied.

It makes sense to me that it is silly having to provide it again even though they received it in the sealed envelope. I always get so stressed out filling anything out for USCIS . Thanks for the advice on reading the forums.

Timeline

We included a copy of the DS-3025 with the AOS packet and they still sent an RFE asking  my husband to provide his medical exam record (which should have been in the sealed envelope he received from the embassy with his visa). He called the panel physician in his home country, but they would not release the records to him. He ended up redoing the exam here in the US and sent it in to our field office. 
You should be fine…worst case scenario is your spouse has to repeat the exam here and send in another sealed envelope.
