I'm a U.S. Citizen filling out the G-325A form, sponsoring my wife for a change of status from F1 visa to permanent resident. My question is, under the "This form is submitted in connection with an application for:", what should I put? I checked "other" and wrote "Petition for wife's I-130". Is that okay? This is an absurdly confusing question I've discovered.

Secondly, within the thick black box at the bottom of the G-325A form ("Applicant:"), I put my name, and under (Alien Registration Number) I put "N/A". That's correct, isn't it?

Thirdly, under my mother's name, should I put her married name and then her maiden name in parentheses like: Smith (Jones), or just her maiden name and nothing else?

You're filing for AOS, therefore for your wife to be a permanent resident so it's not other that you select but "Status as Permanent Resident" (the I-485).

Yes for the black box you put your name and either N/A or NONE since you just don't have one of those numbers.

08.2006: Entered with a B-2 visa.

07.06.07: Civil Wedding

07.17.2008 AOS approved with interview. It took 367 Days!

11.08.08: Big family wedding

09.18.09-10.03.09: First trip to France with Hubby

07.17.10: Card in the mail :) Done until citizenship
