I-130a how to write foreign addresses?

Puea Sub-district, Chiang Klang District 
Nan province 55160

 

How exactly do I write this in the space provided when doing by mail. 

 

Is the city or town considered the sub district or district or are both listed there?

EDIT:

 

          I just realized that I originally gave you info for Pat 4, question 6/7.

          For Part 4, question 13 I would fill it in as:

 

13a: 126/3 Village No.15,  Ratchada Village

13c:  Puea Sub-district, Chiang Klang District 

13d:  Nan

13e:  55160

13f:   Thailand
