adding to I-130 (mistake)

Hello,

 

I already submitted my I-130 a few weeks ago but on part 5 of the application if ask if you ever filed a petition for this beneficiary or another alien.

 

I forgot to add my ex-husbands name and I don't know how to correct it properly.

 

Is there a form I need to fill out or do I add the additional information page to my attachments on my account?

"Forgot to add his name" or checked the NO box?  Not the same thing.  One is an omission.  The other is a misrepresentation. (lie)
