We have an ill mother who just got released from hospital but needs 24/7 care for the next few months. My sister has a visitors visa but just left in Jan after visiting for 6 months. I have a letter from the hospital saying mom needs 24/7 care is there some kind of approval process I can go through so sis can come visit for a few months while she is receiving.

Nope, actually mentioning the nursing aspect is not a good idea. She can try visiting. Could her Mother go home?

She can certainly try, it has been a few months, how long does she think she will need to stay?

All she can do is try, perhaps via Abu Dhabt?

She can try to return to the US for another visit.  There is really nothing you can do.  However, there is the possibility she will be questioned and denied entry.  She should be prepared to convince CBP that she will return after the visit.

I would look to finding 24/7 care within the US.

That is not the purpose of tourist visas.
