NBI Clearance - Advice Needed

My wife just made an appointment for NBI clearance.  I want to make sure we get it all done properly, as far as her names.

 

In the online sign up process, it asks for your maiden name to be the main name.  Then it asks for your husbands surname.

 

My wife was married before so she has had 3 surnames.  Do we need to request that the NBI clearance covers all the surnames?  I recall some discussions on this, but it was years ago, before the online process started.  When she goes to NBI, does she request coverage of all the names?

 

In her U.S. visa documents, I detailed her name changes.

 

Any tips are appreciated.

Yes, your wife must list all the names she has ever used on legal documents.  I suggest listing her passport name + your surname on the Name fields, then all her previous legal names on the AKA field -- previous married name; maiden name; any other names used on legal documents.

 

The NBI staff may want to remove the maiden name from the AKA field, but you should insist on keeping it there.  I recommend bringing a print-out of the US embassy checklist to her NBI appointment -- https://travel.state.gov/content/travel/en/us-visas/Supplements/Supplements_by_Post/MNL-Manila.html#pre_interview_checklist

They have a pdf version of that too that looks real official when printed.

Well, Only In Da Philippines!

 

Went to Olongapo NBI with appointment for AM.  No line outside.  Internet is down and they don't expect Globe to restore it until next week.  Come back next week.  Of course they will have a huge backlog now.

 

Might look for another office not too far away.

NBI Olongapo finally got their internet back and we went today.  My stepdaughter was done in 5 minutes.  My wife, with her 5 AKAs, come back May 9th!
