Medical should be scheduled typically a few weeks before the visa interview.

 

If interviewing at Montreal, biometrics will be taken at the consulate on the day of the visa interview.

Yes, you may apply for police certificate before the I-130 is approved.  Your VJ profile indicates Canada, so I assume you'll be needing a PC from RCMP.  The PC should be sent to you.  You'll need to upload a scan to NVC, then bring the original to the visa interview.
