Adding a child to interview and medical(split)

Have similar situation, have to add child to immigrant visa interview, embassy said they will review the request to add child. The interview date is one month out, how long does it take for them to complete the process and will they give a new interview date? The clinic will not give medical exam date for child without appointment letter. Any information will help. Thank you.
