How to find medical test places?

So i am applying for adjuof status for my parents. I am told medical reports should be sent along with origional i485 application. 

  Where do i find those places, and what kind of paper if any i need to take to them to do the tests? Don't we need to file i485 first and then when they want medical dont immisend some documents to take it to clinics to do medical?

  Do do i just walk in there and have tests done ?

Only the section that the applicant has to fill, the rest is the civil surgeon

No, they will not ask. They just want you to pay
