My wife doesn't know if she passed her citizenship test.

Again, a tier 1 only sees the same information that you and I can see in your case. Sometimes they don't even work directly for CIS and they're contractors. Just like he told told you none was sent, do did mine in regards to the overall status of my case. And he was wrong. 

Hopefully the tier 2 will contact you soon. This time, ask for his name, badge number, and to document anything he says. 

If I were you, in between now and the time he calls you back I would be filing for a FOIA to see what's on your wife's file. But you seem very confident in what the tier 1 said, so...

I don’t have much to add; you’ve already gotten excellent advice from other, more competent members. What I think is crucial though and what can’t be stressed enough is this: stay on top of all of your cases when dealing with USCIS. Don’t assume things just go well and will be taken care of in due time. While that might be the case for some people, there have been many cases where USCIS had to be nudged (or contacted via congressman/Senator) because they just didn’t care that a case was outside normal processing times or otherwise “screwed up” on their part. 

Therefore: like @Rocio0010 said: enroll in USPS informed delivery. Keep track of things like address changes with USCIS (keep confirmation mails and such) and check your status on their online tool regularly. Monitor processing times regularly as well. That way, you can react immediately.

Exactly, and this is the issue everyone here is trying to help you get to the bottom of and resolve. Because if it isn't resolved the N400 will never get approved ever. I'm just keeping my fingers crossed for you (BIG hope) that the best scenario happens and the I-751 gets waived and approved. Then approval of N400 can commence.

So if my wife has to refile an i751 then she can't get citizenship?

First I will address your first question, which is how to file FOIA. Visit that link and watch the videos.

 

Second question is if your wife has to refile 751 she can't get citizenship? Nobody said that. Here's a summary of what has been said to you:

 

Your first 751 is not approved yet and nobody here knows what happened. Not even the tier 1 agent. Apparently the interview was indeed scheduled, but you never received the notice. We are still waiting on either a call back from a tier 2 or a result of a FOIA request to see what's on file for her. It hasn't been denied, but it hasn't moved. Denials and NTAs can take years, so just because it is not denied now it doesn't mean it never will.

 

Your wife filed for citizenship, went to the interview, and was told she couldn't be approved because her 751 wasn't approved. That's true and correct. Officer never said that she can't file for N400 again after the 751 is approved, either the one that's pending on the limbo, or a new one. 

She will be able to file for citizenship once she gets the 751 straightened out. 

Again, as others have said, you seem to be focused on the citizenship, but you have to crawl before you learn to walk. I just don't know how else to say that you have to figure out what perspired with the 751 first, or if you do need to file a new one.

Yeah I have informed Delivery.  Anything is possible, the tier 1 guy could be watching Netflix when I am on hold. He could have been lying when he said that he would be able to see if an interview letter was sent and where it was sent. A tier 2 will contact us soon anyway.

I never said he was lying, or even implied that. I just said they are dangerously optimistic. Just because he can’t see it on his end, it doesn’t mean it wasn’t sent out, because, like I said four times already, he sees the same information you do. 
In the meantime, you have plenty of the most seasoned members out here telling you what to do. These people know how USCIS works better than a tier 1, rest assured.
Anyhow, I’m quoting @Mike E and I’m “Mikeing” now and I’m out. You obviously trust a tier 1 more than what about 8 people have told you. Good luck!!! Let us know how it goes.

I can look at my wife's case by just inputting the receipt number in the uscis web page.  If a tier 1 can only see that information then again that means they are watching Netflix during the multiple holds or just chit chatting with friends.  Maybe he was trying to score with the chick in the next cubicle and i was in the way.  Who knows with those tier 1 guys... I am agreeing with you, he had to be doing something to fill time on the multiple holds I was put on.

The thing that I don't understand is why are you just relying on a letter when you can also check the status online? in fact, if something has taken that long, wouldn't you want to call about it?

Having said that, the officer could have asked about the missed appointment and given your wife the chance to explain why she missed the appointment. Then a combo interview could have been done.

How does one request a combo Interview?

The combo interview would have been at the N-400 interview. The officer would have seen that there is a pending I-751. If your wife passed the N-400, then the officer would have reviewed the I-751.

Well the officer didn't, the officer said he will talk with his manager and get back with my wife in a couple of weeks by letter.  I am waiting on tier 2 uscis call now anyway

Read through the entire thread so far.  Before doing anything else, wait for the tier 2 call back and if that doesn't happen in the allotted time the tier 1 gave you, call them back.  The IO from the N400 interview stated a red flag exists, but don't look at that like the end of the world.  Best case scenario, the I751 is still open, and that package needs to get to the IO that did the N400 interview.  If that happens, the IO could request another interview for the I751 or they could approve it and then approve the N400.  If the I751 is denied and closed, then a new I751 would have to be filed, but I think the N400 would simply stay in limbo until the new I751.  Considering how USCIS functions, I tend to think the I751 is still out there somewhere.
