I won’t venture a guess on that, because if that was the case they would have had to issue an I-751 denial letter at the same time as N-400 was shut down. 
Usually they spell out dates and specifics in the N-400 denial letter ( ex your I-751 was denied on xyz date .. and therefore you are not eligible)

 

The N-400 can be re-opened by the Field Office without an N-336 , you just need to make enough noise before 30 days ( via letter to Field Office Director/service request and senator/congressman/omnbudsman )…and if those don’t pan out a response, file N-336 .

It is a form letter which means his office was not able to help.  That letter doesn't address your issue.

In summary, it took 13 months for approval of the CR-1.  It took 44 months for approval of the I-751.  It took 4 months for approval of the N-400.   It took 172 days from N-400 application to Oath Ceremony.   It took 6 weeks for Passport, then 7 additional weeks for return of wife's Naturalization Certificate..

But reading your posts, the 751 was never denied, right? Your last update was that an interview was scheduled, correct?

Here's what I would do if I were in your shoes since the 751 is not denied yet (at least you were not notified of such denial, were you?)

I would send a package TOMORROW to the field office director, signature required. In that package, I would explain the situation and try to show as much as I can all the efforts that I have done to solve it. Therefore, I would include on top a letter signed by your wife explaining that she went to an N4000 interview at xxx office on xxx date, that she was interviewed by xxxx officer and that on xxxx day she was wrongfully denied. Then I would continue to explain the reason for the denial, stating that she never received a copy of the interview letter for a 751. Therefore, she did not attend such interview. I would also include as many details as possible (hopefully you followed my advice of keeping a log with details of the calls you had with USCIS). I would ask the FO director to look into this situation themselves. Sign every document you send. Make a copy of the package, send it, keep the original with you.

On the n400 denial letter it did state that the i751 denial was because she didn't appear for the interview. But this n400 letter is the first time we knew of a denial.  Will a case officer be able to reschedule an appointment at this point?

No, not a case officer. A field office director would.

 

I think this is worth asking Jim Hacking. He goes live in 20 minutes. 

 

I am not an attorney and this not legal advice at all.

So I would prepare a package like I said, and address it to Mr. Bierman (if I read the name correctly).

1) Send the package I suggested, to Mr. Bierman, with signature required. And I would even label it "RE: appeal 751 and N400 denial). I forgot to add that in that package I would add a screenshot of the status your wife has for the 751, possibly with the time stamp. You can even screenshot the case history, to show that you never got the update "interview scheduled". 

 

Then I would wait about two months, and if that doesn't work, I would file a new 751. Waiting would also buy you time to get the FOIA results.

 

Maybe @Family can tell me if I am wrong in my approach?

Waiting would also mean my wife is here not lawfully after a certain period of time.
