Depends.  The CDC site says this is required if coming from China Macau, and Hong Kong.  I am not aware of the need for those coming from other countries.  That being said, if you are transiting, you need to check that country/countries as well.  My wife recently transited through Dubai, and they require it (although they all but ignored it), but it was not required to enter the US.
