Yes, she applied after the denial of the b1/b2. Unfortunately, she got refused again on her Canadian visitor visa, probably due to the tie with me. However, there is no ties in the USA, so I am confused how can she prepare for the second time of b1/b2 interview?

So, she got denied a Canadian visa, then a US visa, then a Canadian visa a second time?  In what sounds like a relatively short amount of time?  I rarely say this, but there is nothing she can do to improve her chances to get a vIsa for either country right now.  Her multiple attempts, in two countries, presents as a desperate need to get into one or the other -- with far more at stake than just a visit.  It is time to look at meeting in a third country, IMO.

If I recall correctly, most VISA applications would ask if you had been "denied" a VISA from some other country, and not just the USA. These forms talk in USA "legal speak" and there are subtle differences (if I recall correctly, you can be turned away from the USA without necessarily having any VISA denial on record).

I wonder if there was a discrepancy in the answer to this question, i.e. did the applicant say they'd never been denied a VISA anywhere before but the US Consulate discovered the denial?

US dies not ask about denials elsewhere.
