I filled out the initial DS-160 visa application online and asked for an email copy of the confirmation (which never came. I made good and sure it was the correct email. Strange)

My question is: how long does it typically take to get the initial appointment? (I need t owait for that initial appointment to "upgrade" to an emergency appointment for medical reasons..

Thanks much.

1.  You can print out the confirmation page online.

2. You will need to pay the fee and get a receipt.

3.  Typically you will use the receipt to schedule the appointment yourself.

Maybe I didnt type it right on the OP, but I did complete the visa application, and did get the certification of application, but did not get a link to pay, nor an email telling me where to go pay for it.

My understanding is that it needs to get the date assigned, prior to getting the "emergency speed up" (I kinda need my wife here in the states pretty badly, I go in for a super serious and dangerous operation around March 1st (LVAD heart pump)

You will not get a link to pay.  The fee must be paid at any RCBC or you can make an online payment at PesoNet.  Once you have the fee receipt, you go online and schedule the appointment.  You should review the complete "Non-immigrant Visa Application"  process at https://www.ustraveldocs.com/ph/.

It is really pushing it to figure out the process, get a regular appointment, then request and an emergency appointment, have the visa interview, get a visa printed and received (assuming one is approved), travel arranged and completed to the US in less than two weeks.  Hope it works out.
