In summary, it took 13 months for approval of the CR-1.  It took 44 months for approval of the I-751.  It took 4 months for approval of the N-400.   It took 172 days from N-400 application to Oath Ceremony.   It took 6 weeks for Passport, then 7 additional weeks for return of wife's Naturalization Certificate..

Complete and submit Form DS-156, Nonimmigrant Visa Application, together with other supporting materials, such as the letter of invitation from your sick relative and proof of ties to your home country as evidence that you plan on returning upon finishing your stay in the U.S., to the U.S. Embassy in your country. You will find an electronic copy of Form DS-156 in the resources section.

but tourist visa is the wrong one/ they need ds 156

 The online DS-160 application replaced the DS-156, DS-157, DS-158, and DS-3032, which are no longer necessary

I have never heard of a spouse accompanying the USCitizen have issue because the USC needed their care.

 

I imagine they got the standard refusal and will put it down to inability to show non immigrant intent

If instead of OP’s wife the companion was a Nigerian who was not related to OP, the result will be same.  The reason for the result matters not.  
 

Best to focus on defining the actual problem in order get to solution. 
 

Problem: OP requires traveling companion who is authorized to enter U.S. airspace and soil.

The question is: How long will OP need care in the US? Less than 6 months? Less than a year or will OP need to stay in the US for 1+ years? 
If over a year, maybe it is better to apply for spousal visa and look to see if they can DCF
I can see why the tourist visa was denied if the required length of stay was not clear.

I believe this is the reason for the denial. They will need more 6 months. I have not seen anyone in rehab at this level of care for less than a year.

On the contrary I believe applying again and immediately asserting she has zero interest in a spousal visa, chances would be much higher.

 

Sometimes consular offices default to the cookie cutter first option of denying the application without really reviewing the situation.

 

To the OP, I sympathize with your situation. Is your spouse a young lady and have you been married for a relatively short time? If that’s the case her chances may be a little worse.

 

Alternatively you can try your senator/congressman or humanitarian parole however that may take months.

The most pressing thing is to get me to the USA. She wouldn’t have overstated. In any case she didn’t even get a chance to explain. She was asked only two questions and denied immediately without even looking at any documents. 1)?Whats your name and 2) what do you do for a living. 
 

I understand it’s a privilege and not a right but I still can’t shake off the feeling of being let down by my government.

Maybe another question is, are they perhaps considering she’s coming to do a “job” that should be done by a U.S. caregiver in which case it would be illegal? If you’re reapplying perhaps you should reframe it as emotional support.

 

Some of the visa “restrictions” are problematic because they’re not necessarily crafted to cover every possible scenario.

She was not given any opportunity to explain and was only asked two questions, what her name was and where she worked. Thanks.

Timeline

I would try again, but apply for the medical treatment category.  There should be a box to describe the treatment - put down the surgery for the USC spouse and the need to accompany him.  Have the surgery/hospital infirmation as well as a doctor's letter explaining his condition and the need for his spouse to travel with him.

 

If this fails, the only other option I can see is humanitarian parole for urgent medical care.
