this is a repost. i accidentally posted in the wrong forum.

hi friends!

due to the huge backlog at the consulate in montreal, me and my boyfriend want to get some of the requirements done before the consulate contacts us. that way, we can send the packet out as soon as we get it, and avoid further delays. i took a look at the contents of packet 3, but i don't know what any of the form numbers mean except for ds-56 (or whatever the visa application is). could anybody tell me what the other forms are and what the other requirements are? all i know is he needs a medical, a police record, and i-134 (affadavit of support). does anyone know what the specifications are for the police record? is it just one from the rcmp, or one from each town he's lived in? for the medicals--is the list of doctors on the embassy page complete? i'm just trying to figure out if there's a doctor closer than toronto (i don't like driving there i had a panic attack last time ) also, one last question. i am a permanent resident in canada. i have been living here for 4 1/2 years and i am working here. all of my sources of income, bank accounts, etc. are here---none in the u.s. anymore. my mom is willing to co-sponsor for us, and i would like to send the correct forms down to her to fill out right away, so that they are here and waiting once we receive packet 3. i-134 doesn't appear to be for a co-sponsor to fill out, though. is there a different form i need to include?

i have gotten some information about the sponsorship part through some new posts that have been put up. one says that i have to fill out an i-134, and my sponsor must fill out a separate i-134. i also understand that i need to attach my u.s. tax returns for the last 3 years, but i don't recall ever receiving anything from the irs after filing. i know for sure that i didn't get anything this year after filing. can i use my files that were prepared by h&r block or do i have to get in touch with the irs and receive some sort of statement. also, my mom in indiana will be co-sponsoring for me since i have no u.s. income. since she will meet the 125% line with her income, does she still have to include her tax returns?

as for the I 134 , yes he does need a bankletter stating his present balance how much was deposit during the last year and few other questions (my fiance askd at his bank for statment for sponser in immigration porpuses they knew exactly what we needed and prepared the letter with the bank logo on it ),

he will also need a letter from his employer stating his income ,

you also need a DS 230 , and DS 156K including with DS 156 ( you can download that too )

those are the documents the embassy of montreal expects you to send back with the checklist once you resive package 3) ,

as for the medical there will be a list of doctors ( in toronto ) included in package 3 , prices are usualy between 190 - 210 $ you can pay in canadian fonds ,( I calld for my medical wich will be in august here in montreal they told me , to bring the immigration letter , my vaccination booklet 3 passport photos and I can pay the 210 with visa or cash )

about the police certificate , I did some research and found best place to get it ( was also the cheapest and quickest 25$ , 15 - 20 min wait ) is the R.C.M.P office bring your passport and tell them its for immigration purposes to the states , they will do a CIPC ( or what ever the letters stand for , when they do a canada wide criminal check )

well that was the fast version , I probly forgot the list something in there somewhere , but you could always look at the full guide here on the VJ homepage , also if you have trouble completing the forms , the VJ example forms are perfect exemples ( I used them when I got stuck and had no trouble so far ) I wish you good luck in your journey Lona

You need a name check done by the police. Depending on where you live, it could be the Commissionaires, the RCMP or the local city police. Check for your area. Unless you have a criminal record, a name check will suffice. If you have a criminal record, along with any court documents you must bring to your interview, you need to have a fingerprinting check done. This waiting time could be up to 4 months, so best to do it now. Some people have only had to wait about 2. 5 months, but don't take any chances on waiting for this. To close...unless you have a criminal record, just get the name check done.

All forms you need should be here on VJ, so you could get started filling them out now.

As far as the medical goes, a list of designated doctors is here on VJ, as well as the USCIS website. Even if you live in Toronto, you don't have to see a doctor in Toronto. There may be some in the surrounding areas. Look to see where else you may be able to go. Since it's not your own family doctor, you'll be stranger anyway, so it shouldn't matter whether you see a doctor in Toronto, London, Peterborough...or whereever. However it MUST be a doctor listed as designated by the USCIS.
