Once you are back to the US you should file I-90 form asking to replace your card because of mistake in personal details.

 

AFAIK this cannot be done outside of the US. Ideally, you should have checked your details on GC before going abroad.

 

Did you adjust your status in the US or did you get a CR-1 visa in passport based on consular processing? 

 

If you had a CR-1 visa

Do you have a I-551 stamp in your passport which you received after entering the US on visa? Is it still valid? If yes, use it to board the plane and enter the US. Then file I-90 ASAP.

 

If you adjusted in the US

This may be problematic. My understanding is (not a legal advice) , you could try boarding the plane in hopes the airline agent won't notice the mistake on GC. If this doesn't work, try getting to neighboring US countries such as Mexico and cross by land.

 

Either way, there's almost certain chance you're going to be put in secondary by CBP to verify your identify. That's IF you manage to get to the US border.

You've a 2 year gc. My belief is that you if you file I-90 now, given processing times, you will never get replacement 2 year gc with the correct date of birth. Not before you're gc expires, and not before your 3 year extension letter expires.

 

So I would do nothing about it now unless the airline refuses to board you. If so, file I-131a to get a boarding foil or travel to Canada or Mexico and enter via the land border. If at the land border, you can request CBP give you an I-551 stamp. They probably won't but if you don't ask the answer is always no.

 

Once state side, if you want USCIS to give you an I-551 stamp, you will first have to file I-90 (it won't cost anything if it was USCIS's fault). Once you have an I-90 receipt you can request an I-551 stamp. If you get an I-551 stamp, expect to surrender your gc.
