My commission expires on?

Hello All,

At the bottom of the I-134 form (Affidavit of Support) it asks "My commission expires on:". Should the person I am having fill out this form (the sponsor for my fiancé) place a date here? Is this the date at which she will no longer "responsible" for my fiancé as a sponsor?

Two more quick questions:

1. If my sponsor is an American citizen by birth, does she need to supply proof of her citizenship with this form?

2. On question #11 where it asks about the sponsors intention of support, is it looked down upon if my sponsor indicates that she "does not intend to make specific contributions"? (My fiance fiully plans to seek employment upon moving here).

You are referring to the place the Notary signs. The Notary will know what to do with that section.

1. Yes, a photocopy of a "certified copy" of their birth certificate or passort copy.

2. The sponsor can indicate "does not intend". The form is used for all non-immigrant visas. Sometimes the support intention is specific.

Facts are cheap...knowing how to use them is precious...

You need to get your affadavits notarized. the part where it says: "My commission expires on:". is the date where he/she needs to renew there license, so nothing for you to fill out there. That entire part is for the notary.

1. Yes, we did bring a colored copy os her certified birth certificate.

2. No, my mother-in-law did so and we did get approved, no questions were asked.
