Do I need to fill form DS 5507 for my child's passport application?

Hi everyone,

 

I was going  through the required documents for my child's passport application and something is not clear, I am hoping someone here can help.

My child was born overseas but we currently live in the US. She has a CRBA and got her first passport while overseas. Now it's time to renew her passport and DS 5507 is listed under the required documents for applicants born outside the US but the CRBA already states she's a US citizen...I renewed my first child passport a couple of years ago (same situation) and I don't remember that form. And all my research indicates that the form is used to support CRBA applications. Does anyone know if it is required?

You did that already to get the CRBA. 
 

The child has a U.S. passport now. 

 

For evidence of U.S. citizenship goes, the old passport is all that is need to get a new passport.  Leave the CRBA at home in your safe.

  • Full validity, undamaged U.S. passport (expired passports are accepted). Full validity means the document is/was valid for 10 years for adults and 5 years for children under 16.”
