Vaccination Supplement w/o copies of medical???

Hey guys,

We're trying to put together our I-485 and have came across the vaccination supplement. My wife and I went to the consulate doctor 3 days before her interview and he didn't give us a copy of the exam results. He put them in the sealed envelope that was taken to the interview.

So, my wife took the exams, but I think the info was given to the POE immigration (In the brown envelope).

What should we do about the vacc. supplement, because we don't have a copy of my wife's immunizations and she certainly doesn't want to take all those shots again.

I'd heard that we can send in our I-485 and those records will be in the computer and she will receive a letter telling her what shots she needs. Does anyone know if this is true?

Can you clarify what it is you are missing? It seems you are saying you have the vaccination supplement - if you do then that's all you need to send (they have the medical from the embassy) and you don't need another medical at this point. If you do not have the vaccination worksheet (DS3025) - that's what you normally get - then you may have to get all the shots or take a test to see which shots you need (probably both will cost the same!) and then you can get the vac supplement. Maybe if you just do the civil surgeon thing it will be ok

No we don't have the vaccination supplement. I'm sorry, but I'm a little confused with this. Is that all we need to do is get the vaccination supplement? We don't have any records with the shots she's taken, but she took all her shots to enter the U.S. on a K1 visa.

You have several options. The safest thing to do (in other words, least likely to get an RFE) is to try to get the doctor's office in Brazil to give you a copy of what shots you got at that exam, or even better a copy of the vaccination worksheet. Then you would bring it to a civil surgeon here, who would fill out the vaccination supplement, put it in a sealed envelope, and you would send it in with your AOS application.

You might not be able to get a copy of the vaccination worksheet or of the vaccination records. In that case, you could go to a civil surgeon here and either get the shots again (usually only 2-3 shots) to have the civil surgeon fill out the sheet, or have them take blood tests to prove that you're immunized. Then you get the civil surgeon to fill out the vaccination supplement and again send it in with your AOS application.

Another option that some people have had success with is simply sending in the application with a letter stating that vaccinations were done at the full medical and the information was surrendered at POE and hope you don't get an RFE. This way you save money by not having to pay a civil surgeon anywhere from about $20 to $200, but if you get an RFE you will have to do one of the things above anyway and it will delay your application. But some people have sailed through without a vaccination supplement at all, and you might get lucky.

While it's true that our records were surrendered at the POE, I read on the USCIS website that K-1'ers just need to send in a vacc. supplement with the I-485. Does that mean that they already have the I-693?

If that's correct then I'll schedule an appointment only for the vacc. supplements and mail that in.

I have the same situation. I can only assume that my wife had all her vaccinations before she entered the country and was therefore not given any copies of her I-693. Why would one need a "supplemental" to something that needs no further additional information? Or at least that's my logic.

I wrote a letter detailing this anomaly and supplied it with our I-485 package. My case was just transfered to CSC about three weeks ago but so far no RFE's.

Tha VAX worksheet that you should have is a manilla card with DS3025 ( i think) on it. You should have been given it at the POE when they opened the sealed package. This could have been taken to a civil surgeon whoe would give you the I693A suppliment. Many people pass just fine on the DS 3025 and save themseleves the problem of the CS and the suppliment.

If it is possible the easiest way to solve this is to get in touch with the doctor in Brazil and get another DS3025.

As has been said above. If you can't do that then you can either write a letter and take your chances or throw yourself at the mercy of the civil suregeon.

I really don't see any other options. Making sure that you are properly immunized is pretty important to them, and rightly so. They can't just go on your word that you had all the shots.

The vaccination supplement is the I693! So don't confuse yourself.

If you want the I693 you will

1) Need to produce the DS3025(vaccination worksheet)

OR

2) Get the shots again!

So! You can get the shots again (not a big deal really) or get a test to determine which shot/s you need etc. or ask your Barazilian doctor for a copy of the vaccination worksheet. Good Luck

Thanks guys for your help!

I think now the only thing we can do is write a letter (accompanying our I-485) stating our situation of Pamela not receiving any copies of the medical and it given to the Immigration when we arrived in Miami. When we arrived at immigration in Miami we were given nothing but Pamela's I-94 stamp and received no manilla folder.

I'm almost positive the doctor in Brazil doesn't have a copy (my wife asked him for one and he said we wouldn't need it, because the sealed envelope we took with us to our interview to the consulate).

So the letter I think would be the best idea stating what has taken place and hopefully we won't get an RFE. If we do get one then we'll have to deal with it then.

It's like Clint Eastwood said, "Do you feel lucky?" Well let's hope so! :D

The vaccine supplement is the I-693A - not the I-693. Even if the I-693 is in the sealed medical envelope, it is not possible for the I-693A to be in there as that is a form used only by US doctors.

Best bet is to call the embassy doc and ask SPECIFICALLY for the DS-3025. If they grumble at you, tell them you need it for further immigration processing. If you can get it, then proceed on to a US civil surgeon for completion of the I-693A at what should be a nominal fee of $50 or less.

This is still a 'grey area' with AOS procedure. Some people get by with doing nothing - others get RFE's. Even if you don't get RFE'd, if the adjudicating officer is unsatisfied at the time of the actual interview, you can be approved for the green card but sent away with instructions to get the necessary vaccines.
