as far as I know all forms were sealed in a big envelope which included the visa in passport and another big envelope which was to remain sealed.

That sealed envelope was handed over to the immigration at the airport here in USA. The contents of the envelope contain medical, police clearences, and lots of other stuff, but nothing was ever returned.

So I am lost to what a DS-3025 is or how to get a I-693

DS-3025 is one of the 4 forms that the Panel Physician fille out at the visa medical exam. It is the form that documents the visa applicants vaccination history.

You should have been given, or you should have asked for, a photocopy at the time of the exam.

Not really!

You can go to a civil surgeon and he may be able to determine what you need and fill out the I-693.

Probably will require a complete medical tho and cost some bucks.

Call a few DIFFERENT ones and see what they have to say.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship
