Birth Certificate for Canadian Passport

I tried posting this question in the Canada forum but got no response.  I'm just hoping someone here might have an answer.

 

I'm Canadian and a permanent U.S. resident.  Recently I lost my Canadian passport, so obviously I can't go back to visit family until I get a new one.  I also lost my original birth certificate.  Just wondering if the certified statement of birth I found amongst all my immigration paperwork would work for my passport application.  On it says "Certified A True Photostatic Print of a Record" issued by Ontario Office of the Registrar General,  and shows my Statement Of Birth.

You should be able to check on the Canadian passport site

Dual US/Canadian citizen here, I've gone through this process before.  If the Canadian passport that you lost has not expired, and if you know the passport number, date of issue, and expiration date, you can get a new one with the passport renewal form, a birth certificate is not required.  If the lost passport was expired, or if you don't know the passport number and dates of issue & expiration, you will have to apply for a new passport and include an original birth certificate, which you can request online from the province of your birth's vital records department.  A photostatic certified copy is not acceptable for a new passport.  For the renewal, or for a new passport if the one you lost has not expired, you will also need to include a separate, completed declaration form for the lost passport.  You can mail your application from the USA to the Gatineau Quebec address.
