Question abt I-134 Form

Hello,

Which I-134 form do we use:

1)---The one provided on this site, or the USCIS site (they are both the same, with OMB no. 1615-0014 Exp 04-30-07 Rev 02-28-07??

2) or the one I received in the Packet3...which is differnt from the ones above:

OMB No. 1115-0062, Rev 12-1-84, and no exp date given. also doesn't have whole "intro" like the above ones do, just starts off with the questions and filling out part.

and this one, is with me, in my packet3. do i TCS it to my fiance??

The USC sponsor can use any I-134 they want. They fill it out, get it notarized and send it to you with the supporting documentation.
