Just recieved Packet 3 - very confused

Hi everyone,

I just recieved Packet 3 from Montreal and I have a few questions, as I'm sure everyone does when they recieve their packet.

1) Can I complete forms DS-156K and DS-230 online, or do I need to use the forms that were included in my packet? If I can complete them online, where can I find them?

2) The checklist says to arrange for a medical appointment. How can I arrange for this when I don't know when my interview is going to be? I thought the medical takes place after recieving packet 4. Can I just check this off and deal with it when I get packet 4?

3) If I get my 5 passport photo's done now, will I just have to get them done again when it is time for my interview, considering that the interview wait time is over 5 months right now?

Any help anyone can provide is much appreciated!

They are all vaild for a Year so you can go ahead and get them done now. I would call the Doctor's office and make an appointment and send in the checklist and not wait until after the medical as you will lose more time that way.

Just one more thing, I thought that there is a form that I recieve in Packet 4 that I am required to take to the medical with me. If this is the case, how can I see the doctor now? Or am I misinformed about the medical form in packet 4?

You take a sealed envelope from the Medical to the interview. You need to leave enough time after the appt with the Dr. before your interview as it can take time (depending on location) to get results back , and in some cases get the medical courriered to you. I did the medical inbetween Pkg 3 & 4 because i had to travel to a different province and arrange all that. The only thing you could take out of pkg 4 with you to the medical is the date of the interview.. but they will accept the letter in PKG 3 (from my experience), that says you are going for a US Visa. Just check with the Dr's office - they will tell you what you need to bring for proof that you really do need the medical

Use the forms that Montreal sent to you ... I think that they are particular. Complete the one that they tell you to online as it produces a bar code. Complete the checklist even if you don't have all of the information yet .. you have more than enough time to complete everything before your interview. Mail back the checklist and the completed online form ASAP as when they receive this they will put you in line for your interview by this receipt date.

As for your medical ... you know that your interview will not be for a few months after they receive package 3 .. so my advice is to wait a bit before booking it (unless you forsee any possible delays ... see other posts about these delays). The documentation from Montreal told me to get 5 passport pics done .. three would be required for the medical and two required for the interview. The medical only required one (would have saved me $30 as I paid $15 each for a set of two for me and my son).

The DS 156 you defenatly have to fill out on line

the rest you can fill out on line as well ( its actualy better so you can print 2 copies 1 for the interview 1 for your records ), here is the link where you can get all the forms you need

Dr. Seidens in TO will ask to see your letter from Montreal where it has your MTL#. I had the medical done (booked 2 weeks in advance) and had the results within a week.
