Quick question on DS-260

Hello , I am going for an IR1

 

The form asks if I ever applied for a US visa? I personally did not but as a child (ages 10-13) I did live in the US as my father use to work there, I have no details about the visa he was on but my assumption is I would have had something. 

 

What do I put for that question? If I put no and my name shows up in the system, could that be an issue?

 

At age 13 we returned to Canada and have lived here since.

Answer "Yes".  In the explanation box, it's ok to be honest that you don't know the details as you were a child at the time and that you are just assuming that you did have a visa that your parents applied for you.
