Name Change -Information required

We are trying to file for AOS however getting mixed information about the documents that needs to be provided .

First of all we can not seem to find the Form for Name change. We even visited the Probate Court to get the form however we were told that they do not have it and that we need to contact some attorney for the same !

Does anyone know where can we get the form.. ??

Is Marriage Certificate good enough for AOS ?? its only been a month since im here so im nt yet attached to any of my husband's bank , insurance records.

What kind of documents is required to get my name attached to his Bank account ...??

Can we apply for AOS without the " name change " document ?

I would greatly appreciate any information .

P.s : Trying hard to meet the deadline of July 30th.

The only name change document you need is your marriage certificate....... there is no other form... marriage certificate is good for the bank too...

Marriage Cert is fine for AOS

SS Card, along with ID, and perhaps brith cert may be needed to add you to a bank account.

You can file for AOS without a name change doc, in most states the marriage certificate IS tha name change doc.

If you want married name on the green card fill out I-485, and G-325A with married name, and put Maiden name in the "Name on I-94" field on the I-485, and "Other names used" field on the G-325A.

I was told that theres a form ( aka petition for name change ) to fill up for " Name Change " in order to go thru the proper legal way for complete name change ..

I could be wrong ....

There are forms and processes for name changes, but they vary from state to state and in some cases (like in Massachusetts) county to county. If you do a Google search, you should be able to find the form and more info or perhaps another Vj'er who is in the know will kindly post it here.

For us, we are going through the name change process now. We weren't ready to do it at the time we got married, so our wedding certificate has my maiden name. Now, we are going through the process via the forms and the Probate Court as required by the county we live in.

Wow, I've never heard of it - I thought you could just start using your spouse's name at amy time after the marriage I guess, it varies by state...

I did not have to file any additional forms to change my name after marriage.... it is one of the very few times you can change your name without having to go through the courts....

This sounds really good & positive to me...i already have my last name changed on my Marriage Certi...so i guess i will just continue doing the same in all the other forms.
