1.  I'm floored that any refereed medical journal would let this get through, but perhaps the tide has turned.

2.  The author is right about everything (see 5).

3.  The proofreading is atrocious and embarrassing, distracting from the points being made.

4.  References should be listed in numerical order of their citation in the text, not alphabetically by lead author.

5.  The author is fortunate to be retired, or he'd also be dog meat.
