Adoptions and disclosing previous name

I was adopted at 12 by my stepdad. Other than a SS card I never had any other documents in that name. The addition is sealed and my BC states my stepdad as my father and has my new name (middle and last were changed).

 

Do I need to disclose my name change?

 

If so do I need to provide documents?

Most adopted children never knew or met their biological parents , so adoption laws extend to all adoptees. Records are sealed and new Birth Certificate issued . 
 

Your adoption is treated no differently, though you are privy to all of the information of your biological parents. Family adoptions are a lovely act intended to forge strong bonds .
 

You do not ever need to disclose birth names ( most don’t know them) as aliases.
