Yes, his birth certificate is a US one. He was born and raised in the US.

So, as an example, if he was born with the last name 'Smith' and then had it changed to 'Stewart' when he was adopted by his step parent, and then changed it back to 'Smith' after his step parent and mother got divorced, does he have to provide proof of name change?

I would not have thought so, depending on the ages that he had the temporary name.   US immigration forms do ask for 'aliases,' but if he was a minor at the time, and no legal ID documents were ever made in that name, I can't see why it would be needed.

Because he went to court to do a name change post adoption, he does need to disclose the previous name ( adoption name) as an alias .
He can just add the clarification of having been adopted by stepfather and name change at divorce …there shouldn’t be any issues/RFEs .

But it’s a good idea for him to just get court copies of the post divorce legal name change just to have .

This because once am adoption takes place, there would need to have an reversal court procedure to undo it…but one can do a name change only..

If the name changed occurred as a part of the divorce proceedings, all you need is a copy of the court decision that says he could resume his former name.  Adoption records are closed records and will not be obtainable.

The only name change in divorce proceedings that can change was mothers , not OP’s . He would have either had to separately reverse /revoke adoption or file just for separate name change

But all he really needs to do is list alias ,,.so they can run name check. Does not need to document anything to USCIS in their petition

I concur.  He must list the adopted name when asked for other names used, but because his current name matches his birth certificate, there's no need to provide documentation.

I used a step father's name on school records but was not adopted.  The divorce occurred before I had a driver license or any other ID, so I never even listed it as another name used.  Since he WAS adopted, I would list it for sure.

The discussion of divorce proceedings and name change came from OP.  Hence the use of IF....
