USA PR holder travel to Tulum, Mexico

Hello Members,
 
My husband and I are planning to go to Mexico and I am still an Indian passport holder and permanent resident of the USA(2 years green card), initially came on K1.
Questions:
-Would it be okay upon entry to the States to have Indian passport in maiden name and PR card in married name along with having the marriage certificate?
-Anyone with similar situation, did you require any visa for Mexico or any tourist card? From what I read online, visa is not required for green card holders but says about having a tourist card. Is tourist card same as FMM and is it to be filled online or in the flight? Wanted to check if someone has more details.

In our experience: 

 

* You don’t have to pay for or fill out a tourist card when traveling by air or cruise ship to Mexico.  I presume the cost of the tourist card is built into the fare
 

* LPRs are exempt from visas for Mexico.
