N/A in blanks?

Did you all put N/A in all the blank fields that weren't filled out? Thank you!

You can. I put N/A only on the first row of a section and left the others related to that section blank. 
For example- 

Child 1

Name: N/A

Then left all other fields blank

 

Child 2

Name: N/A

Then the rest blank

and such.
