Husband has different names on docs for i751

My husband has two different last names. He’s a Mexican so his name in birth certificate is ex: John Doe smith but his SS number and passport his name is only John Doe. Most of the documents im submitting has his name John Doe Smith on them except our taxes and marriage certificate and our daughters birth cert. this has always been a problem when I submit docs, he’s been using his John Doe name officially since he grew up but for some reason when he goes to DMV to get his drivers license, his name appears to be John Doe smith since they follow what’s on his birth certificate not his Social, and everything follows, our bank account, his docs at work, our bills since I think he uses his drivers license as an ID. I know it’s a complete headache, I want to blame my mother in law lol but I just want to be sure the immigration doesn’t think The docs im submitting are from two people. 
 

my question is, what name should I put on the form? Is it John Doe or John Doe Smith?

Title says your form is I-751, so I will assume husband is the USC spouse…and so best to just ask him what he prefers and go with that…and you will be perfectly fine. Whenever space permits you should always add  “ John Doe” AKA “ John Doe Smith…on blank last page of the form

 

His Proper Real Name is the one on his birth certificate, of course, with the DOS APELIDOS ( maternal and paternal)…but he seems to have given up using the DOE SMITH a long time ago. It gets challenging to fight for corrections, space if both apelidos are long such “ Hernandez Gutierrez “ ..
