Hi Everyone.

I am planning on filing I-130 for my mother next month. As a single mom, what are the documents I needed to file for the I-130. Meanwhile, my last name differs from her last name, took my last name from my dad's side although my dad has never been in my life since after I turned 4yrs. Please, anyone with necessary information on documents that will aid me start this process and make it easy, I would greatly appreciate.

Thanks in anticipation of positive response.

What matters is whether your mother's name on your birth certificate matches her name on her birth certificate and/or her passport.

Check the official I-130 form instructions here -- https://www.uscis.gov/sites/default/files/document/forms/i-130instr.pdf

Thanks Chancy for your response, it was very helpful. Meanwhile, the online application didn't show any place to upload the copies of my mother's birth certificate and passport data page. Are they not required when applying online?

They are not required at the petition stage, whether applying online or by mail.  Her birth certificate and passport bio page will be required at NVC stage after petition approval.

Thank you Chancy once again. Do I need to notarize any affidavit of support. This is for the column that asked to provide school document and census with parent name. Since I don't have those, am asking some one to write an affidavit.

Affidavit of support (form I-864) is also not required at the petition stage.  It must be submitted at NVC stage.  No need to notarize.

If you mean 3rd-party affidavit letter, that's not required either.  School document and census record also not required, unless you have no birth certificate that lists your mother.
