Is I-693 required for K3? Or I-693A?

Timeline

Hi again everyone. I'm still trying to figure out how to file an AOS for my wife.

I carefully read through all the paperwork this weekend, but I can't figure out what is needed in terms of medical forms. It says that if you're applying for K1/K2, you can just submit an I-693A, but it doesn't mention K3.

My wife had her medical in February 2007, and I assume her records are on file with the government. She didn't receive any paperwork from the medical, other than her x-rays and whatever was in the sealed envelope that she turned over at the POE.

Can someone please help me as to what is needed for K3? It would be MUCH appreciated!!

I130:

08/10 - Mailed to VSC

08/14 - Received by VSC

08/19 - Received NOA1 (I-797C)

12/13 - Approved!

12/18 - Received NOA2 (I-797C)

I129F:

08/22 - Mailed to Chicago

08/25 - Received by Chicago

09/02 - Received NOA1 (I-797C)

10/23 - Transferred to VSC

11/01 - Received by VSC

12/13 - Approved!

12/18 - Received NOA2 (I-797C)

12/18 - Transferred to NVC, then to Embassy

02/01 - Received Packet 3

02/02 - Medical

02/06 - K3 Interview

02/14 - Received K3 Visa!!!!!

Just the vaccination supplement. There is another reference on the USCIS site that specifically says that K3s do not need to do another medical. I'm trying to find the link now.

I have one other question. Is there an "official" vaccination form that she needs to bring to the designated civil surgeon with her vaccination record? Or will her general doctor's record suffice?

Timeline

Depends on the CS but a general doctor's record should suffice. I brought a copy of the DS vaccination worksheet that I was given after my foreign medical AND all documentation from my doctor in Canada regarding vaccinations and blood antibody tests. The civil surgeon I went to just based it all off my DS vaccination worksheet but I had my other documents with me just in case. Some civil surgeons are picky so bring everything you have in regards to vaccinations past and present.
