When does his Medical expire?

@aaron2020 has listed the reasons not to - when you activate the PR, the expectation is your husband will be a resident of the USA.  The information is shared between the two countries.  Depending which province you're in, he could lose provincial health care benefits right away.  He will have to file a US tax return despite earning his income in Canada.  It's doable, but messy.  Easier just to tie up the lose ends and cross.

Perhaps.  Depends on the couple's priorities, which is easier FOR THEM.

I suspect you've never had to deal with the disorganized hot mess that is the Canadian Revenue Agency.  The IRS part is a cakewalk by comparison.

Nevertheless, this is a decision make based on the couple's priorities.  Considering all the angles is good though.
