How are denials conveyed to the immigrant after admin processing?

Lets say you weren't approved at the interview and the consulate said they needed more info before making a decision. After you send in the docs and lets say you are denied due to an inadmissibility (say past fraud finding). How is this denial conveyed to the intending immigrant. Do they send you an email or they ask you to come back in without you knowing you are going to be denied. Or will the status on the ceac status checker say denied?

Timeline

Varies.  Sometimes you pick up your passport, it has no visa, and instead there is short explanation for the denial.
