Covid-19 Vaccine is not listed on DS-3025

Hi all,

 

I got Covid-19 vaccinations and completed DS-3025 in my home county for K1 Visa without any issues before I came to the US.

However, there is no area on the form to mark that I received the vaccination. Do I need to go to physician and ask to fill out Part 10 Vaccination Record on Form I-693  or can I just provide my Covid-19 vaccination certificate from my government?

When my husband had his medical in his country before the K1 interview the doctor wrote it on the bottom of the DS-3025 form that he completed the Covid vaccine. Did you look on the bottom of the form or on the back if it was written in? There isn't a box they can select for it.

Thank you for your reply. There isn't note about Covid-19 on my form.

I asked the hospital and they said they cannot write about the covid-19 vaccine because it is not required to be proved for K1 visa.
