Once the expedite request is approved, probably around two weeks maximum. I have, though, seen them be even same-day in some rare cases. But I think the 45 day window they give you is a highly conservative estimate. Do be aware that this only expedites the USCIS stage; NVC will remain at 3-4 weeks standard for your case number, and then interview availability is subject to the local embassy's conditions.

Is your fiancé going to apply Schengen visa?
