I read the letter stating that April 27th my case can resume after one year of negative drug test. The drug test started in October. My question is how does this process work? After April 27 the medical personnel  automatically reach out to the embassy. And then the embassy reach out to me? Or do I have to initiate some action to reactive my case? Also will I have to pay for my interview and medical again? If everything go as planned how long would it take for the visa to be issued?

You should contact the consulate after the 1 year requirement to ask for instructions.  No one will "reactivate" your case for you.  You will be required to undergo and pay for a new medical, and other fees as directed by the consulate.  Good luck.

In summary, it took 13 months for approval of the CR-1.  It took 44 months for approval of the I-751.  It took 4 months for approval of the N-400.   It took 172 days from N-400 application to Oath Ceremony.   It took 6 weeks for Passport, then 7 additional weeks for return of wife's Naturalization Certificate..

Please post the steps once you have some.  Our date is not until June 1, I’ve been searching high and low for steps after the one year ban, but nothing.  I’m really hoping there won’t be another interview.

Only YOUR consulate can give you proper instructions.......I'm 99% sure there will be another interview.

Did you call St Andrews as recommended?

Do you have proof of one year of negative drug results, then you can contact embassy and schedule another medical exam.  Without the one year proof, you have to start taking drug tests every 3 months for the next year.

The ban started in April 2021 the first drug test was scheduled in October at Andrew medical and he been randomly going there since . My original question was when does the year start from April or October? But I did tell him to contact the consulate by email. So we shall see.

The ban would start in April, it starts from the date he did the last medical

Did the letter that you receive from the embassy have a date that That you can resume your application? I contacted the embassy last week and I was told that the ban ends on that date.

Quick update:  The refusal letter that we received a year ago stated that his ineligibility expires on June 1, 2022.  My fiancé was contacted for another random drug test on May 31st.  I’ve been emailing the embassy since June 1st.  Finally today someone called my fiancé from the embassy and asked when was his last drug test.  They told him that once they receive the results from the hospital, he will be contacted.   About 10 minutes after that he received another call telling him that they will email some instructions.  The email contains a letter to schedule a new medical exam, he also need to submit his passport using the courier service.  
 

There is also a child (K2) involved.  He was told that the child will complete a medical after.
