One Health Pass

My wife is travelling on her passport that is still in her maiden name. We have not done ROM. Tickets purchased to match the passport. What name should she register under on One Health Pass...married or maiden?  Her vax docs and pcr test are both under married name. I'm guessing we want to match the passport and ticket? Thanks in advance.

I'd say passport name, since OHP is for arrival and easier to match this with arrival record.  Just take with you your foreign marriage certificate as supporting document for vaxxcard and test.

I didn't encounter any issues because all my docs were in my maiden name. The documents were however checked at every step of the way: once at domestic flight check/in, again at intl flight check-in and again on arrival in Manila.  I hope your wife won't have issues with her test results and vax card being in her married name.

 

Worst case scenario, if they don't accept her PCR results, hopefully her airport of departure has an onsite clinic where she can get an antigen test with results in 15 minutes.

My wife had zero issues. Well, besides finding Bradley International Terminal at LAX))). Absolutely pain free. Thanks, A.
