I immigrated to the US when I was 4 and my social security card had only my Chinese name. My aunt gave me an American name and somehow, my legal name is now my American name and Chinese name combined. I never had my name legally changed but I just started adding my American name to every document I had. I really don't know how this happened because I replaced my social security card at some point and it now has both of my names. My passport as well. My brother and mom who immigrated with me only has their Chinese names on their legal documents. 

 

I got my Certificate of Citizenship when I was 13 and it only has my Chinese name. My certificate was stolen and I need to replace it. I forgot the date it was filed (maybe I can call the UCIS office closest to me?). The N-565 form also asks the name the document was under. I will have to use the Chinese name or else I don't think they would be able to find it in the system but I'm concerned there might be an issue because it doesn't match the names on all of my other legal documents. Any idea on who I can ask about something like this?
