Hi all! So, I had my medical done at my home country - London. I opted to get my vaccinations done overseas and after I was done with the whole medical examination, they gave me what I’m assuming is the DS-3035 sheet for vaccinations highlighted with what I needed to get done overseas, but I’m hearing that people also got something called the DS-2054? I’m not sure what that is but I did not receive one of those and was wondering if I was meant to? Or would that have been put in the huge envelope I received a few days later? Also, was I meant to receive anything else because I just have the DS-3035 page. If anyone could help I’d be grateful :)

You probably got a copy of the DS-3025 vaccination worksheet.  The DS-2054 is the medical exam report and should be in the "Do Not Open" packet to be handed to CBP at POE.  Visa applicants typically get a copy of the DS-3025 only, for their personal records.

 

In your case, because you opted not to complete the required vaccinations at or prior to your visa medical, the form that you will need for your adjustment of status is the I-693 form signed by a civil surgeon in the US.

Thank you so much! When I get the I-693 done they only fill the vaccinations part in right? Because I had already done the whole medical & the vaccinations is what I’m only missing. Also, when I get the I-693 done, do I include the copy of the DS-3025 saying what vaccinations I needed to get done? Or do I just send the I-693?

It depends on the civil surgeon.  Many will not sign the I-693 if you do not re-do the medical exam.  You may search for a civil surgeon who will not require you to undergo another medical and will charge only for documenting vaccinations, but that is up to their discretion.  That's why VJ members recommend K1 applicants to complete their vaccinations at or prior to their visa medical, to avoid that hassle entirely.

No need for DS-3025 when you submit the I-693.

No need for DS-3025 when you submit the I-693.
