Well as stated on this visa instruction form on the embassy, click here it is saying that the applicant must complete form DS-156 and DS-157 online. Do they mean complete the electronic version or can the downloaded version be use.

Can someone who has received packet 3 post a scan or list the approved doctors for the medical in Thailand? I also am interested to know if there are any options for the medical outside of Bangkok.

Well, now that I know that an approved doctor is closer to where I am, I want to get the medical done ASAP. So I want to get my fiance to go for the medical before packet 3 arrives. A few questions:

1. A previous post referenced the online forms being incorrect. Assuming the doctor doesn't have the correct ones, Is the only way to get the correct forms by receiving packet 3?

2. Would it be correct to say that the only thing she needs to have for the medical is the new case number from the NVC? Will the doctor require anything else from her? Anything in packet 3 needed?

3. When the medical is finished, Do I pick up paperwork from the hospital and take it to the interview? Do they send anything directly to the embassy or do I have to bring everything? Does she see all the results?

Correction: I meant to say this in the previous post. I do know that she needs to bring the forms, her passport and 3 1X1.5 inch photos. As well as money for the fee. Anything else needed?

1. Yes, if the doctor doesnt have the forms, it would be best to wait til you receive packet 3.

2. the only thing needed from packet 3 is the medical forms. Im not sure if the case number was printed on my fiancees medical docs received in packet 3.My fiancee did bring her passport and 2 photos.

I say just call the Doc. first to see if they have all the forms necessary. If not, like matt said it's best to wait for the packet to arrive.

3. The medical facility will give your fiancee a sealed envelope and told not to open it. She will not know the results. This envelope is given to the CO on her interview day.

You can also ask the nurse who take care of you to photocopy the result for you .. and you should at least have the result (copy) for your vaccination so you can use it for your AOS also (i forgot the name of the form) But if you ask them nicely they will likely copy it for you before they sealed it..

PS.. at Bumrungrad that is..... and dont forget to check your name on everything.. mine was mistaken put in Mr. instead of Ms. and we need to take it back to correct it (i dont know if it's bad but i dont want to make mistake ;p )

K-1 = 4 months

AOS = 5 months

I-751 = almost one year

--- There are 4 forms needed for the medical examination. In the K-1 packet 3 website of the Embassy, there are only 3 forms on the web, which are DS-3024, DS-3025 and DS-3026. The missing one is DS-2053 which can be found in K-3 packet 3 website 0f the Embassy. I think you can just download and print out the forms and take them to the hospital with you.

--- She doesn't need a case number from NVC. Also, the case number is not printed on the forms. She need only the forms, her passport and one any-size photo. As well as money for the fee, as you said.

--- Correct!!

I second that you should wait until the packet 3 arrives. Everything will be told in the packet 3.

Perfectly helpful information. We are a few hours from Chiang Mai and we may be needing to go there early next week. That is the reason I would just like to get the medical taken care of. I don't think I will have packet 3 by then. I feel emboldened by all the great advice so I think I am just going to download all the forms and get it done. If anything is not in line with packet 3, then I will have time to schedule another trip to Chiang Mai, but the information given sounds very sound and complete. I'll take a gamble so I may not have to travel twice and update all on how it goes.

I also can't believe she has to go to Bangkok to personally hand in the paperwork for the police certificate. That is much further from us and is going to be an expensive day trip.

Also, congratulations to Matt on the successful interview. How did it go???

hey thanks, I wasnt with her for the interview, but she said it went very well. The CO just asked her where i was from. She said USA. He laughed. then he asked her how we met and how many times ive seen her this year and that was it. He said that its approved, signed something, and told her to come back the next day at 2pm to pick up her passport and visa. :) Dont sweat it, you guys will do fine. But now that its over, it feels weird not worrying about anything. I feel like i need to be worrying about something. But oh well, their is always AOS to look forward to worrying about. :lol:

Don't forget to bring her ID card (they will ask to see)

Yes, she has to come by herself unless she is staying overseas

Note: she can ask to mail her police certificate when it is complete
